"The Alchemist won’t take my ingredients." Solution: You picked them up in the wrong order. Drop the Bitter Nettle, flip to male, pick it up again. Then flip to female to pick up the toadstool. The quest checks your current form at the moment of acquisition.

"I accidentally killed the ghost’s locket quest giver." Solution: You can’t. The ghost is invincible. However, if you attack it 100 times, it triggers an easter egg where it calls you a "menace" and gives you a piece of coal. That coal is used for a different, hidden quest (The Miser’s Gift).
